Below is a listing of the "required documents" to enroll in Saints Peter and Paul

Catholic School for the 2008-2009 school year.

                                

                                Required Documents

 

ATTENTION:  Those entering the sixth grade

Health records must be updated for all students entering the sixth grade in the fall of 2008.  All immunization records and sixth grade physicals (this does not include the sports physical) must be at the school by opening day.

The state of Kentucky mandates the following health requirements for students entering the sixth grade in 2008-2009:

1.    A physical within six months on a School Medical Examination Form – Sixth (6th)  Grade Form (KDE/MIC approved 1671-410)

2.    Two doses of MMR

3.    Three doses of hepatitis B vaccine.  Be advised…

a.     there is a required waiting period between each dose

b.      to receive all inoculations will take six months.

4.     Tetanus Booster

Please contact your family physician, the Christian County Health Department, or Blanchfield Army Community Hospital and take care of this necessary task as soon as possible.
